Report an LSEMS Employee

The Los Santos Emergency Medical Services department holds our employees to the highest of standards and take all reports and evidence very seriously. This form is used to file a complaint against an employee(s) of the Los Santos Emergency Medical Services who the citizens of Los Santos feel have not lived up to those standards.
Please fill out the following form as best that you are able. All complaints will be reviewed by the Internal Affairs division of Los Santos Emergency Services.
You are required to provide any and all evidence of the claims you state as part of this submission. The more evidence you have to back up the claim, the more likely our investigators will be able to conduct a full and thorough investigation. Note that complaints for petty issues may be rejected, as will those that do not provide sufficient supporting evidence, at the discretion of the LSEMS IA Department, or LSEMS High Command.
Please note that we only take reports of incidents that have occurred in the last month ((1 week)). Understand that in many cases, the results of the investigation may not be shared with the complainant, as the privacy of our employees remains paramount; however if you do need information regarding an investigation, please speak to a member of Internal Affairs, or a High Command member, who will determine what can be shared with you.
